The Center for Alcohol & Drug Treatment is seeking a full-time (0.75) Pharmacist as a part of a Victory Fund Grant. This position guarantees 2-years of employment as a part of the Victory Grant, with opportunity for continued employment based on grant renewal. CADT is looking for an individual with an interest in working with individuals struggling with Substance Abuse and can provide care and compassion to our patients. The will work directly with patients and be provided with a part-time assistant (Medication Navigator) to assist them in their endeavors to improve patient health. We are looking for the Pharmacist to start the first week in July, 2024. Summary The pharmacist will provide leadership for expanding clinical pharmacy services (comprehensive medication management and medication consultation) and education across CADT's residential inpatient treatment programs and office-based opioid treatment (OBOT) in collaboration with the inter-professional team to meet desired outcomes and measures of 2-year Victory Fund grant (see attached). Responsibilities: Meet 1:1 with patients with substance use and co-occurring conditions to comprehensively review their medications for appropriate indication, effectiveness, safety, and ability to adhere to their regimens ("Comprehensive Medication Management" (CMM)/"Medication Therapy Management" (MTM)).3-5 Identify medication-related needs of patients served by CADT inpatient residential programs 6 (Howard Friese Lakeside/Prindle, Marty Mann) and office-based opioid treatment (OBOT) program 7 (moving into Urgent Care Behavioral Health at HDC 810 4th street, formerly the Clarity Project) and develop & implement shared solutions that account for social determinants, available resources, and are person-centered. Document, bill, and provide follow up evaluation. Educate patients/patients individually and in group settings about pharmacotherapy for addiction and other co-occurring conditions that are commonly experienced by community members (infectious disease, contraception, depression, anxiety, insomnia, pain, PTSD) that is built on trust and respect. Co-develop curriculum with inter-professional team. Empower patients with skills and tools for interacting with the healthcare system that enables them to engage effectively with practitioners and advocate successfully for their unique needs and social contexts. Collaborate internally and externally with primary care/mental health clinicians to carry out therapeutic plans and ensure follow through and coordination among providers involved in an individual's care plan. Track required data/metrics and develop a future plan for pharmacy clinical services following the grant period that accounts for sustainability in our existing payment system and in a transformed payment system. Required Qualifications: BA/BS degree plus at least eight years of experience, or Advanced degree plus at least six years of experience, or Terminal degree plus at least one year of experience to include a Bachelor's degree, or Doctor of Pharmacy degree from an accredited school of pharmacy Completion of residency (PGY1 or PGY2), or at least 2 years of experience as a practicing pharmacist in an outpatient clinical setting Minnesota pharmacist license in good standing Ability to travel to multiple work locations Commitment to providing collaborative team-based care to diverse populations who have comorbid substance use, mental health, and medical diagnoses Demonstrated experience in, and an ongoing commitment to working effectively with and across diverse communities Capacity to work in a highly matrixed work environment, ability to cope with complexity and ambiguity, and creatively solve problems Preferred Qualifications: BCPP Experience at an outpatient, ambulatory care clinic, mental/behavioral health clinic, or substance use treatment clinic Experience with clinical program evaluation and reporting Supervisory experience Experience working with low-income and/or ethnically diverse populations Our Benefits: Health, Dental, & Vision Insurance 401K (Up-to 8% employer contribution) Paid Time Off & Development Time 10 Paid Holidays Short- & Long-Term Disability (No-cost) Accident & Critical Illness Insurance Life Insurance On-the-Job Training Tuition Assistance Programs Employee Assistance Program (EAP) HSA Eligible Health Plans Family and Medical Leave Bereavement, Jury Duty, and Military Leave We prefer to promote internally and promote opportunities for development.The Center for Alcohol & Drug Treatment completes background checks in compliance with Minnesota Department of Human Services.
