Your responsibilities include: Enhance cognitive, psychosocial, and vocational interventions that empowers patients receiving physical rehabilitative care to achieve an optimal level of overall functioning Cultivate independence and personally fulfilling, socially-emotionally meaningful, and functional effective interactions Establish patient goals to facilitate independence, social, and psychological adjustment Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care Make recommendations on transitional care or outpatient services after short stay Develop intervention plans, and share ideas about network management issue Facilitate and assist leadership in quality improvement initiatives and risk management Represent TeamHealth in engaging, establishing, and maintaining relationships within the community Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees Timely response in chart completions Why TeamHealth: At TeamHealth, our purpose is to perfect our physicians' ability to practice medicine, every day, in everything we do.
