The Transition Teams are county-wide teams that offer a service to young people from the age of 16 who have lifelong disabilities. The Role A Local Authority in West Susinteraction is looking for a Social Worker to join their Transition Team, Lifelong Services. This is for a full-time, permanent position. The Transition Teams are county-wide teams that offer a service to young people from the age of 16 who have lifelong disabilities. This Local Authorities Lifelong Services Teams have a unique focus, and their Social Work Practitioners are exceptional at supporting the teams to deliver legislatively compliant services. They offer a dedicated, strengths-based approach to disability that we know makes a positive difference. The successful candidate will have a challenging, complex caseload, and provide an accountable social care service to those people who access or need the service. The successful candidate will be expected to work within organisational, policy and legislative requirements which include the Care Act, Mental Capacity Act and the Mental Health Act. Good working knowledge of safeguarding, risk backssment and enablement are essential. This Local Authority is committed to improving their services for the children and families and currently in a key time in their improvement journey. This journey includes establishing a new and experienced senior leadership team, driving their ‘Children First’ improvement plan and implementing the Family Safeguarding Practice Model.
