Forensic Coordinator: Serves as primary clinician/case manager for NGRI consumers released from CMHIP, either designated as Community Placement clients, or Conditionally Released clients. Maintains regular communication and liaison with FCBS team. Complies with all Community Placement and Conditional Release Policy requirements, including completion of all required paperwork. Prepares Quarterly Reports on each client's progress for FCBS Team and the courts. Continuity of Care Coordinator: Oversees referrals from inpatient facilities wanting to place non-residents in our community, often to an ACF or Nursing Home. Reviews clinical justification and appropriateness of the referral. Determines legal status (certification and/or court-ordered medications). Confers with facility slated to receive the client in terms of ability to safely manage the client. Reports information obtained to Medical Director and Director of Intensive Services. Essential Duties: Completes on-line training, and complies with individual training plan if required. Participates in supervision by coming prepared with an agenda. Reports high risk/problem cases, and utilizes a problem solving approach as well as feedback. Attends supervision at times and intervals agreed upon with supervisor. Submits 90% of all Progress-to-Date forms within 3 working days. Completes 95% of all paperwork by the due date. Engages consumer and family in the treatment process and goal-setting. Identifies risk factors including lethality. Works constructively with consumer to reach agreed upon outcome, and coordinates care with internal and external providers. Maintains appropriate professional standards and provides appropriate follow-up for consumers. Maintains a professional attitude and conduct. Maintains effective interpersonal relations with consumers, all Jefferson Center employees, and the public. Effectively responds to client/consumer needs and problems, initiates and maintains positive interactions, timely response to phone calls, pages, email and other requests. Works cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteers to work on Center committees and/or task forces. Forensic Coordinator Specific Duties: Provides clinical services to assigned consumers, including ongoing assessment of risk and compliance with terms of release. Prepares and submits timely quarterly reports to FCBS and the DA. Incorporates conditions specified by FCBS and the court into Jefferson Center treatment planning process. Facilitates communication and coordination between FCBS, the legal system, and Jefferson Center Reports any unusual incidents, special circumstances, or clinical decompensation to FCBS. Ensures lab tests for medication compliance as required and obtains copy of results for consumer records. Continuity of Care Coordinator Specific Duties: Becomes proficient in understanding OBH Continuity of Care Policy Serves as initial point of contact for inpatient facilities wanting to discharge non-residents to facilities in the Jefferson Center service area Gathers current clinical information and past history Determines legal status, including certification, involuntary medication, and current court venue. Reviews referrals of clients from other mental health centers of non-resident clients into Jefferson Center Service area. Confers with potential receiving facility regarding appropriateness of referral and the facility's ability to safely manage the client. Education, Knowledge, Skills & Experience Required: Master's degree in counseling, social work, psychology or related field required. 2 years related experience and/or training required. Licensure preferred. Salary Grade 51 - Master's Unlicensed Clinician $47,600 to $60,400* Salary Grade 52 - (Licensed Clinician LCSW, LMFT, LPC) $57,000 to $72,700* Additional Salary Information*: All salaries at Jefferson Center are calculated based on years of total relevant experience.* The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.* Additional pay provided for evening, overnight, and weekend shifts.* Master's Bonus LPC/LMFT Bonus LCSW/LP Bonus $1,500 1 month anniversary$1,500 1 year anniversary($3,000 Total)$1,000 1 month anniversary$1,000 6 month anniversary$1,000 1 year anniversary($3,000 Total)$2,000 1 month anniversary$1,000 6 month anniversary$1,000 1 year anniversary($4,000 Total) Additional Bonus Information*: Based on full time (40 hours/week).
