Role Overview - Through The Gate Worker: In this inspiring role, the Through The Gate Worker will establish partnerships with agencies for supportive meet-and-greet services, assisting users to transition into rehabilitation or community-based treatment through solo work and public transport. Collaborating with Health and Wellbeing Practitioners, they'll manage referrals, backss needs, and ensure smooth post-release support, including linking to treatment and mutual aid like AA/NA meetings. Responsibilities – Through The Gate Worker: Develop links with local housing providers to minimise the number of service users released without a stable residence. Coordinate with medical services as needed, to ensure service users have access to essential medical care upon release their release.
