At the Royal Borough of Kensington and Chelsea, we work collaboratively to help local residents achieve a great quality of life. From housing to healthcare, we collectively strive to adopt the most relevant and cutting: edge approaches to dealing with every person and every situation. We employ a variety of systemic interventions from a trauma informed perspective. The Family Services team exists to provide an exemplary service to care leavers. Across a spectrum of ages, we take on full responsibility for young people between 18:25 to give them the best possible opportunities as they transition from care to independent living. Where you come in is making sure that this transition is smooth and successful. The Role: First and foremost, as a Care Leaver Personal Advisor, you'll take a leading role in developing realistic pathway plans for young people. Through providing bespoke services that suit specific needs, your day: to: day will involve forming and maintaining relationships with care leavers. Your efforts will not only contribute to the development of our services but will allow for pathway plans to cover every aspect of an individuals life. On occasion you will participate and lead in group work. With a focus on accommodation and independent living, you'll manage a broad caseload and liaise with the wider Family and Children's services and partner agencies where appropriate. Due to the sensitive nature of this role, you'll need to participate in regular team meetings, be up to date with all relevant training materials and receive regular supervisory support from your manager.
