Description The Community Support Professional III, provides direct intensive community crisis stabilization and prevention supports to individuals diagnosed with behavioral health diagnoses served by the REACH program. Also assists with the development of and staff training in individualized behavior support plans. Works as part of the REACH team to ensure services and resources are coordinated among providers as needed to help individuals served live and participate optimally in the community. Starting Salary: $42,952-Depending on Experience$2,500 Sign on BonusResponsibility * Works with clinical staff on REACH team to ensure completion of required assessment of needs and strengths to meet the admission and ongoing provision requirements for services. Ensures the proper pre-authorization documentation to qualify and receive payment for all services rendered are submitted within allowable time frames and maintained in the record. * Works with team to write effective and measurable Individualized Service Plans (ISPs) as required upon admission, as the needs of individuals served change, and at least once annually thereafter. Works to ensure supports and interventions are provided as stated in the plan. Ensures all assigned plans are reviewed at least quarterly. * Promotes independence by teaching and modeling life skills such as: personal hygiene, nutrition, understanding of mental and physical health and well-being, methods to access assistance, financial management, coping skills, self-esteem, and self-reliance. * Provides leadership and training to direct care and service provider staff to ensure proper implementation of support plans. * If assigned to work at the Crisis Therapeutic Home (CTH), correctly administers medications to individuals supported as prescribed and in accordance with all medication administration regulations. Provides assistance with other medical procedures (i.e.: blood sugar levels, wound care, etc.) as ordered by a physician and approved by supervisor. Completes a Medication Administration Incident Report Form by the end of shift for any problems that involve medications. (Note: Part Time/Relief staff will be trained to administer medication based on program needs. Staff not formally trained and certified by NRVCS may not administer any form of prescribed or over the counter medications.) * Collaborates with the individuals' support team and NRVCS programs to ensure that all services reflect the individuals' needs. Monitors satisfaction with services provided and helps individuals served complete satisfaction surveys as needed. * Provides education to individuals served and families/care givers to promote better understanding of behavioral health and developmental disability related topics. Works to establish linkages between individuals served and naturally occurring supports in their family and community. * Maintains complete and accurate medical records and completes all administrative documentation according to policy and procedures. Maintains all communications, both written and verbal, with respect and confidentiality and complies with HIPAA and 42-CFR Part 2 standards and regulations. * Ensures all safety, health, and universal precautions are properly addressed and monitored. Completes NRVCS Incident Report forms by the end of the shift for any observed or suspected incidents that involve individuals served or staff welfare and/or safety. * Assists with crisis resolution and general problem-solving. Ensures services are provided with respect to the dignity of the individuals served as outlined in the human rights regulations. Also reports any observed or suspected violations of the human rights regulations according to NRVCS Human Rights policy. * Promotes positive communication and collaboration among team members and other community resources needed to provide quality services. Conducts self in a professional manner at all times. Participates in team meetings unless absence is excused, completes proper written communication and relays other helpful information as necessary by voice mail and/or e-mail. Serves on designated teams, committees and work groups. * Stays informed and up to date on operating policies and programmatic procedures which promote quality services and recommends changes when needed. * Assists with admission process and discharge planning to and from private and public psychiatric hospitals and training centers, including providing support and input to the emergency services providers and judicial system during commitment hearings, when appropriate. * Provides back-up for co-workers, and covers after-hours crisis stabilization on-call services as assigned. Must be accessible by telephone after hours. If assigned to CTH, works with supervisors and team members to ensure adequate staff is available as needed at the CTH facility at all times. Arrives at work on time and remains at work site until coverage has arrived for the next shift. Provides on-site orientation for new staff, students and volunteers as assigned by supervisor. * Travels to other locations as required and provides safe transportation according to NRVCS policies and guidelines as needed to perform job duties. * Works with team to ensure all services are provided at the expected levels of quality and are in compliance with all applicable agency policies as well as any local, state, and federal requirements. * Performs at the expected levels of individual and team productivity. Ensures all client records are in compliance with applicable local, state, and federal requirements including guidelines as they pertain to DBHDS Licensure Regulations, Waiver, State Plan and Clinic Options Medicaid. * Ensures all required certifications and/or licensure are kept up to date. Attends/provides other training as required to meet personal professional development and/or program needs. * Performs other related duties as assigned by supervisor, service line manager and/or director. Qualifications * Knowledge and training in best practices for providing community based behavioral health services for individuals with disabilities residing in community settings. * Thorough knowledge of DSM -V etiology, symptoms, treatments and prognoses * Knowledge of local, state and federal regulations of laws governing commitment of persons to institutionalized care, and standards and related ethical and legal guidelines including Medicaid, and other payment sources. * Ability to work independently and part of a team and work effectively with other agencies and develop community resources. * Ability to communicate effectively orally and in writing and to prepare and present educational materials. * Must possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment. * Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment; * Must have acceptable drug screen results upon hire. Education/Experience Required: * Master's Degree in social work, psychology or human services related field is required. * Virginia Board of Counseling registration as a Qualified Mental Health Professional-Adult (QMHP-A) and/or Qualified Mental Health Professional-Child (QMHP-C) status is required. Additional compensation will be considered for any new employees who speak multiple languages.
