Reliable transportation Valid Drivers license Must be at least 21 years or older Personal cell phone Work Schedule We are looking for an exceptional disability support professional with a minimum of 5 years of community disability service's experience to assist a male brain injury survivor successfully move forward in their recovery. Position is Monday thru Friday 8 am to 4pm Description: SLG's Disability Support Professional Will supervise an assigned brain injury survivor living in Bristol, Connecticut, to develop and maintain independence in the community, participate in volunteer activities in the West Hartford area, attend medical appointments across the region, and provide educational interventions where needed. Associate degree in healthcare related field required, Bachelor's level applicants encouraged to apply. Are you ready to join our team?
