Community Health Worker - Physicians Reach Out Summary: Care Ring - a fast-growing and highly respected community organization seeks a passionate individual for the role of Community Health Worker (CHW) for A Guided Journey program. The CHW provides case management, coordination of care and health education to A Guided Journey program participants. As a trusted member of the community, the CHW will facilitate access to services and work to provide high quality and culturally competent service delivery. The CHW must have a passion for Maternal-Child Health to support improved birth outcomes, and family health well-being. The goal of this role is to increase perinatal health knowledge and self-sufficiency among our participants. We do this through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. Job Functions Provides care coordination and linkage to community resources to program participants and their families. Interviews, assesses needs, and refers patient/families to resources for social, emotional, or financial assistance when appropriate and serves as a patient advocate to help in the decision-making process. Screen for depression and refer to community agencies as necessary. Demonstrates open and respectful communication in all interactions with program participants and their families. Maintains accurate records of program participants and submits information in a timely manner to meet grant productivity requirements. Maintain HIPAA and confidentiality. Complete all training and ongoing professional development for CHW certification. Attend meetings, events and conferences. Follows policies, procedures and best practices of Care Ring and Physicians Reach Out. Participates in community outreach activities. Performs related duties as assigned or required. Skills Maintains knowledge of Physicians Reach Out program. Documents all patient interactions, assessments, interventions and referrals in a timely manner. Makes decisions independently and as part of a team using sound analytical and problem solving skills. Work effectively with culturally diverse clients and colleagues.. Participates in staff meetings and team meetings. Utilizes effective communication skills to collaborate with the multidisciplinary teams. Develop and evaluates client progress toward goals and documents outcomes. Performs related duties as assigned or required. Develops and maintains relationships with community agencies and partners. Initiative-taker, able to multitask independently, and exercise sound judgment and decision-making. Requirements Proof of two Covid-19 vaccinations is required as a condition of employment High School Diploma or GED Consideration/preference will be given to job-related education (i.e., certifications, etc.) Excellent written and verbal communication skills Basic computer skills Must be able to work a flexible schedule including some evenings and weekends Bilingual-Spanish Knowledgeable of services available in the community preferred No calls, emails, or visits please .
