The successful candidate will be required to work 37.5 hours per week, working from Monday - Friday, 9AM - 5PM. Temporary cover is required for approximately 6 months, with the possibility of extension. Please note: this role and involves supporting clients in the community and from the service in Redbridge. The successful candidate will be required to work 37.5 hours per week, working from Monday - Friday, 9AM - 5PM. Temporary cover is required for approximately 6 months, with the possibility of extension. Please note: this role and involves supporting clients in the community and from the service in Redbridge. In this position, you will be expected to; - Hold a caseload of 12-15 clients and carry out regular key working sessions resulting in agreed SMART Support Plans which are reviewed regularly and support the individual in rebuilding their life away from the street. - Complete comprehensive risk and needs backssment- Work alongside the street outreach service to build and develop collaborative networks within the rough sleeping sector. - Co-ordinate support for each individual from first contact through to them achieving a more settled and fulfilled lifestyle. - Utilise personalised budget funds to promote routes away from rough sleeping and support meaningful activity away from the street setting- Work with specialist teams and other projects to support rough sleepers and maximise their opportunities. - Advocate for clients and enabling them to access appropriate services and housing- Work flexibility to meet the needs of clients by completing street-based work alongside outreach teams. - Support clients to access support services including mental health and substance misuse services, or services for those involved in the criminal justice system. - Support rough sleepers who are street active, rough sleeping, in emergency & TA accommodation or in homeless shelters Please note, this role involves loneworking.
